慢性痛のメカニズム

Mechanisms of chronic pain.

慢性痛は，急性痛と較べて，そのメカニズムはとても複雑であり，人間における科学的データが乏しいため，推測や仮説に基づいて論ぜざるを得ない。まず，慢性痛とはどのように定義し得るだろうか？　私は「疼痛システムの異常に基づくか，または，疼痛システムが，慢性的に駆動されることによる痛みである」と定義している。ここでは，治療上の見地から，便宜的に“peripheral”“peripheral-central”，“central”と“psychologic”に分けて論じていくことにする。慢性痛のメカニズムとして特徴的なことは“psychologicまたはenvironmental factor”によっても生じうることであろう。

　I described some possible mechanisms of chronic pain. Because there are only a littel human scientific data, I review some speculations and hypothesis.

　I define chronic pain as pain due to chronic stimulation of pain system or disorder of pain system itself. Mechanisms of chronic pain are more complex than that of acute pain.

　Characteristic of mechanism of chronic pain is that it can be caused by psychologic or environmental factor.
