成人てんかんの難治化―側頭葉てんかんの難治化要因

What factors make temporal lobe epilepsy intractable?

はじめに

　てんかんの難治性は，実際には発作だけの問題ではなく，精神症状の有無や，認知機能の水準，最終的な社会的適応性などをも考慮に入れた，個々の患者の処遇困難性のことであると考えた方が，より日常臨床の実感には則している。しかし，こういった本来の意味での難治性はあまりにも広範な問題群を含んでおり，到底，この誌面での展望は不可能であるし，清野（1994）も指摘しているように，てんかんの難治性は発作自体を指標として語られることが一般的である。そこで，今回は，発作の治療抵抗性に問題を限定した上で，代表的な成人の難治てんかんとされてきた側頭葉てんかん（ないしは複雑部分発作：以下CPS）に的を絞り，その難治化の条件を中心に文献的な展望を行った。このテーマは，すでに，本邦では福島ら（1993）が詳細に論じているが，手術適応の可否をも射程に入れながら，この誌面を借りて再度この問題を取り上げることは意味のないことではないと考える。

Some reports of prognosis of complex partial seizures or temporal lobe epilepsy are derived from clinical series not limited to newly diagnosed cases. The assessment of the prognosis from such studies is mostly dismal ; cases with complex partial seizures entering in remission are less than 40% in these studies. This is far below the remission rate in other types of partial seizures. On the other hand, more than 50% of newly diagnosed complex partial seizures are controlled. This remission rate is a good match for that of other types of partial seizures.
