眼瞼運動の高次障害：開眼失行，閉眼失行およびその類縁症候

Supranuclear disturbances of eyelid movements: Apraxia of lid opening, apraxla of lid closure, and allied disorders

はじめに

　失行症は，Liepmannの定義によると，“過去に集積された経験と訓練により得られた，記憶の必要な運動”の障害である^29）。この定義は，1世紀前のものではあるが，現代の失行論でもそのまま通用している。また失行症では，障害が不特定の神経支配筋・筋群に現れ，錯行為（parapraxia）が特徴的異常であるという点も現在の失行論にそのまま引き継がれている^55）。また随意運動と自動運動の解離（automatic-voluntary dissociation）も失行症における運動障害の特徴であると考えられている。確かに随意運動と自動運動の解離は失行症においてみられる特徴的異常の一つではあるが，失行症にのみみられるものではなく，これを示す運動障害のすべてが失行症であるわけではない。しかし，この点に重点を置いて，反射性・自動性運動が保たれているのに（麻痺がない証拠）随意運動ができないものを失行症であると考え，“失行”を冠された症候が存在する。

We discussed eyelid movement disorders of supranuclear origin, including apraxia of lid opening, cortical ptosis, apraxia of lid closure, and excessive eye closure. Although some of these disorders have been erroneously called as “apraxia” in the literature because of conceptual misunderstanding of apraxia, they should not belong to the category of apraxia according to both classic and current definitions of apraxia.
