着衣失行―認知障害との関連も含めて

Dressing apraxia

はじめに

　脳損傷による着衣の障害は19世紀末のJackson（1876）の論文や20世紀初頭のWendenburg（1909）の論文に記されているが，独立した症状（apraxia for dressing：着衣失行）として記載したのは，おそらくBrain（1941）が最初であろう。彼は日常場面で衣服の着脱に障害を呈した症例を報告し，後に衣服と自己身体の空間的形態の関係が障害されたためと考えた。一方，着衣失行の独立性を疑問視する見解も少なくない。本稿では着衣失行（広義のものを含む）について，概略を述べ，純枠例を呈示し，他の高次脳機能障害との関係や局在について解説を加える。

Although dressing disability due to brain injury had been reported by Jackson (1876) and others, dressing apraxia that is proper to the right brain damage was first described by Brain (1941). Diffi-culties in dressing apeared to be due to confusions regarding top and bottom, back and front, and right and left with reference to the garments. It has been explained that coexisting other higher cortical dysfunctions, ie, constructional apraxia, ideational apraxia, unilateral spatial neglect, contribute to difficulties in dressing as well.
