磁気脳波（MEG）の臨床応用

はじめに

　神経細胞の活動に伴って脳内には電流が発生する。通常の脳波electroencephalography（EEG）は脳内電流が頭皮上に作り出す電位差を測定する。一方，基本的な物理法則として電流が流れればその周囲に右ネジ方向の磁界が発生するが，脳内の電流もまた周囲に磁界を発生する。これを頭皮上から測定するのが，脳磁図あるいは磁気脳波magnetoencephalography（MEG）と呼ばれる検査法である。MEGの測定はきわめて微弱な信号をとらえることであり，近年の超伝導を用いた特殊な装置の開発により可能となった。MEGは高い空間分解能と高い時間分解能の両者を持ち合わせた優れた検査法であり，将来はEEGと相補的に利用されると考えられる。本稿では，最近臨床応用化されつつあるMEGの検査項目の中で，とくに脳外科手術に際しての脳機能マップという観点での解説を行う。ただし，てんかん外科治療のためのMEG診断に関しては，著者らの他の論文^1-5）を参照されたい。

Our recent clinical experience of the whole head magnetoencephalography (MEG) is summarized in this paper. We applied a multimodal functional brain mapping technique, using a newly developed helmet shaped 64 channel MEG system. This system enabled us to measure evoked magnetic fields over the entire head simultaneously. MEG is completely non-invasive. The whole head MEG is suitable for patients with severe neurological deficit because of a short measurement time. Signal sources, localized by a multiple current dipole model, were superimposed on anatomical MR images of each patient.
