難治性疼痛に対して，大脳運動野電気刺激療法（EMCS）が保険適用になっているが，開頭術を必要とし，手術をためらう患者もいる。一方，反復経頭蓋磁気刺激療法（rTMS）はEMCSの非侵襲的手法と言ってよい。rTMSの除痛効果はガイドラインでも認められている。しかし効果は一時的であり，レスポンダーの割合は20〜50％である。レスポンダーの患者を特定し，繰返しrTMSを行えば治療となり得る。われわれは簡便にrTMSを繰り返すシステムの開発を進めており，2015年12月より医師主導治験を開始した。

Abstract

Electrical motor cortex stimulation (EMCS) for the treatment of pain is approved and supported by health insurance in Japan. However, EMCS requires a craniotomy, and some patients hesitate to undertake EMCS. Repetitive transcranial magnetic stimulation (rTMS) is a non-invasive method of pain control, and the efficacy of rTMS has been evaluated in several studies. However, the effect of rTMS is temporary, and the response rate is 20-50 %. Therefore, it may be possible to treat intractable pain by selecting the responders and repeating rTMS. We are developing a convenient system to repeat rTMS. In December 2015, an investigator-initiated trial began with a new rTMS machine.
