糖尿病と脳梗塞

Diabetes and Ischemic Stroke

糖尿病は冠動脈疾患，脳卒中の確立した危険因子であり，血管合併症として大血管，微小血管の障害をきたす。脳卒中は日本人の死亡および寝たきりの原因として重要な位置にあり，糖尿病患者では脳卒中発症リスクが対照の2倍以上となっている。糖尿病における種々の代謝異常は血管を構成する血管内皮や平滑筋細胞の機能障害を引き起こす。特に，虚血性脳卒中では入院時の血糖値が予後と相関し，全身血管の合併症が生じやすくなるため，その予防には抗血小板薬のみでなく降圧薬やスタチン製剤などの全身のリスク管理を含めた治療が必要である。本稿は糖尿病と脳卒中に関して，その疫学，病態生理，治療の観点から概説する。

Abstract

Diabetes mellitus is an established risk factor for coronary artery disease and ischemic stroke, and can induce micro- and macroangiopathy. Stroke is strongly associated with severe disability and mortality in Japan, and the risk of stroke is 2-4 times higher among patients with diabetes than among matched controls. The abnormal metabolic state in diabetes can cause arterial dysfunction, and patients with diabetes require treatment for each metabolic abnormality to attenuate atherogenesis. Moreover, diabetes affects stroke outcome as well. This review on diabetes mellitus and ischemic stroke discusses three main topics: epidemiology, pathophysiology, and treatment.
