フルニエ壊疽を疑った陰嚢皮下血腫の1例

A case of scrotal hematocele:difficult to differentiate from Fournier's gangrene

　症例は69歳，男性。会陰部の腫脹を主訴に受診した。肉眼的所見にてフルニエ壊疽を疑い緊急入院した。しかし，血液検査所見で炎症所見を認めなかった。画像検査所見にて肋骨骨折による皮下出血が陰嚢を中心として会陰部にたまり，血腫をもたらしたものと判明した。入院後，保存的治療にて症状は消失し，退院した。

A 69-year-old man visited our hospital with the chief complaint of the scrotal swelling. At first glance, Fournier's gangrene was strongly suspected, he was admitted in an emergency. But blood examination did not indicate the inflammation or infection. CT revealed subcutaneous hemorrhage due to a rib fracture which was caused by fall down in acute alcoholic toxication. It seems the hematoma went subcutaneously down to the scrotum. After the admission, with only conservative therapy the symptom subsided.
