心タンポナーデにて発見された腎癌術後再発の1例

Renal Cancer Presenting as Cardiac Tamponade

　52歳，男性。1994年9月左腎癌に対し，腎摘除術を施行した。1999年7月腹部膨満感を主訴に受診した。心エコー検査にて心タンポナーデと診断された。エコーガイド下に胸骨下心嚢ドレナージを施行した。CTにて多発性の肺転移，縦隔リンパ節の腫大を認めた。腫大する頸部リンパ節に針生検を施行した。病理組織像は腎細胞癌と同様であり，腎癌術後再発と考えられた。

A 52-year-old man with a T3a G2＞G1 renal cell carcinoma of left kidney, which had been treated by left nephrectomy about 5 years previously presented with the chief complaint of abdominal fullness. Echocardiography revealed massive pericardial effusion with features of cardiac tamponade. Under the echocardiographic guidance hemorrhagic peri-cardial fluid was aspirated which revealed class V. The next day echocardiography showed no evidence of tampon-ade. CTscan revealed multiple lung metastatic tumors and mediastinal lymph node swelling. Needle aspiration of cervical lymph node showed renal cell carcinoma.
