真菌球を形成した膀胱真菌症

Fungus Ball in the Urinary Bladder:A Case Report

　83歳，男性。8年前より糖尿病にて経口血糖降下剤により加療を受けていた。1995年末に難治性の膀胱炎を来したため当科に紹介された。初診時に尿路感染症と大量の残尿があり，腹部超音波検査で膀胱内に浮遊する多数の固形物が認められた。尿培養でCandida　albicansが同定され，膀胱真菌球症と診断した。治療は抗真菌剤の投与と残尿に対して間欠自己導尿を行ったところ，膀胱内の浮遊物は消失し，尿路感染症も軽快した。真菌感染による膀胱内真菌球の形成症例は，本邦の報告では第11例目にあたると思われる。

A-83-year old man suffering from diabetes mellitus was referred to our hospital with complaints of pollakisuria and difficulty of urination. He had severe pyuria with identifica-tion of Candida albicans. Transabdominal bladder ultra-sonography revealed several ball-like bodies as large as 3cm in diameter. We diagnosed those bodies to be fungus balls in the urinary bladder accompanied with diabetic neurogenic bladder.His bladder infection was cured after the treatment of administering fluconazole in two weeks with bladder irrigation at clean intermittent catheterization.
