精細管内悪性胚細胞の1例

症例は16歳，男性。主訴は無痛性右精巣腫大。右精巣腫瘍の診断にて高位精巣摘出術を施行したが，病理組織診断は精細管内悪性胚細胞で他に精巣胚腫瘍を認めなかった。精査にてStage 1と診断し，現在経過観察している。本症例は，精細管内悪性胚細胞で他に精巣胚腫瘍を伴わないケースであり，調べえた限りにおいては本邦3例目に相当する。
A 16-year-old man visited our hospital with the complaint of painless enlargement of the right testis. Right high orchiectomy was performed under diagnosis of right testicular tumor. Pathologically, the tumor showed ITMGC, which is important according to the hypothesis that germ cell tumor has intratubular origin.
ITMGC without germ cell tumor is very rare, and our case seems to be the 3rd case reported in the Japanese literatures.
