　筆者らは泌尿器科領域における難治性瘻孔および縫合不全に対してフィブリン接着剤を応用し，その有効性を確信したので報告する。

　長崎大学医学部附属病院泌尿器科において難治性瘻孔の4例，縫合不全の3例の計7例に対し，フィブリン接着剤を瘻孔および死腔に直接注入，または再縫合時に縫合補助を目的に使用した。7例中6例（86％）が著効であり，1例（14％）がやや有効であった。また，7例とも再発や合併症は特に認めなかった。

　これより泌尿器科領域における難治性瘻孔や縫合不全に対しフィブリン接着剤は副作用もほとんどなく安全に使用でき，また早期治癒が期待できQOLの点からも，非常に有用である。また腎移植患者や糖尿病の合併例などの創傷治癒機転の遅延が考えられる症例にはぜひ試みるべき治療法と考える。

We report on clinical application of fibrin glue in urologic surgical complication. We applied fibrin glue in 7 cases, 4 with intractable fistulas, and 3 with postoperative wound diastasis. Fibrin glue was directly injected into fistulas and diastasis space of postoperative wound. The overall success rate was 100%, 86%, excellent and 14% fair. All cases had no complications or recurrence. We suggest that application of fibrin glue is safe and useful in urologic surgical complications, especially in patients with poor healing such as renal transplants and diabetics.
