側射レーザーファイバー前立腺切除術と経尿道的前立腺切除術の比較検討

COMPARATIVE STUDIES ON LATERAL FIRING LASER ABLATION OF THE PROSTATE AND TRANSURETHRAL RESECTION OF THE PROSTATE (TURP)

　側射レーザーファイバーによる前立腺切除術と従来から行われている経尿道的前立腺切除術を比較し，両者の手術侵襲，副作用，有効性，前立腺肥大組織に対する変化を比較検討した。

　1993年4月からl1月までの間にレーザー切除術を施行した症例は27例，経尿道的切除術を施行した症例は32例である。年齢，前立腺重量などは両者の間に差はなかった。手術時間，出血量，副作用，在院期間で前者に優位であったが，術後1か月までの排尿状態は後者に優位であった。しかしながらさらに2か月まで経過をみると．両者の有効性に差はなかった。MRIによって両者の組織変化の差，治療経過の相違が示された。

Lateral firing laser ablation (Ultra Line) and traditional transurethral resection of the prostate (TURP) for patients with benign prostatic hyperplasia (BPH) were comparatively studied on invasiveness, side effects, effectiveness and histological changes to the target tissues.

From April to November, 1993 27 patients with symptomatic BPH underwent the laser ablation therapy and 32 patients underwent TURP. Age and prostatic volume were not significantly different in these two groups.
