盲端逆Y尿管の1例

A CASE OF INVERTED Y URETERAL DUPLICATION WITH A BLIND-ENDING BRANCH

　16歳，男性．右下腹部痛を主訴として来院，諸検査により，主訴は右重複腎盂の上極側の水腎症によるものとわかったが，その検査中，偶然に左盲端逆Y尿管を発見した．右腎上半部分切除術を施行したが，左逆Y尿管は症状を欠くため経過観察とした．逆Y尿管は非常に稀な尿管奇型であり，本邦では自験例が6例目の報告となる．また，盲端逆Y尿管としては世界で5例目の報告であると思われる．

A 16 year-old boy was admitted with the complaint of right lower abdminal pain. Excretory urography demonstrated a double pelvis on the right side, and an inverted Y ureteral duplication with a blind-ending branch was coincidently found on the left side. Enhanced CT revealed hydronephrosis of the upper pole of the right kidney and an inverted Y ureter with a blind-ending branch on the left side. A cystscopy showed one normally located ureteral orifice on each side. Left retrograde urography revealed the same abnormality as excretory urography. The patient received right partial nephrectomy for hydronephrosis.
