クラインフェルター症候群に発症した前立腺癌

PROSTATIC CARCINOMA OCCURRED IN A PATIENT WITH KLINEFELTER'S SYNDROME

　57歳，男性．頻尿，排尿困難を主訴として受診．直腸指診にて前立腺癌を疑い，針生検にて低分化腺癌の診断を得た．性腺機能低下所見がみられたため染色体分析を行ったところ47,XXYを示し，クラインフェルター症候群であることが判明した．クラインフェルター症候群に前立腺癌が発症することは極めて稀であり，本邦では本症例が第2例目にあたる．その理由について，男性ホルモンを中心に考察を加え，前立腺癌発生機序について検討した．

A 57-year-old male was consulted to our hospital, complaining of urinary frequency and difficult voiding. His prostate was enlarged to a soft-ball size, stony hard and irregular on the surface. The needle biopsy of prostate showed poorly differentiated adenocarcinoma. The chromosome analysis indicated 47, XXY. Prostatic carcinoma in patients with Klinefelter's syndrome is very rare. This is the second report in Japanese literature.
