胸腔内尿貯留を伴った悪性リンパ腫

A CASE OF MALIGNANT LYMPHOMA WITH URINOTHORAX

　症例は56歳，男子。左側腹部痛の精査中，左腎腫瘍を疑われ入院した。入院後左胸水を認めたため胸腔穿刺を行ったところ尿が得られた。静脈性腎盂造影，逆行性腎盂造影，CT検査より，後腹膜腔腫瘍のため尿管狭窄，腎盂破裂をきたし，更に腎盂外に溢流した尿が胸膜腔に浸潤し胸腔内尿貯留を形成したと判断した。疼痛軽減のため，先ず腎周囲のドレナージをおこなったところ胸腔内の尿が消失した。全身状態の改善を待って腫瘍摘出術を施行した。腫瘍は左腎門部下方より出て尿管を巻き込み腎下極に浸潤していた。さらに空腸部腸間膜，膵尾部にも腫瘍を認めた。病理学的にはB細胞型非ホジキンリンパ腫，LSG分類上び漫性大細胞型であった。胸腔内尿貯留を形成したという報告は少なく本邦では本例が3例目である。

The patient was 56-year-old man with a complaint of left flank pain. DIP showed dilated left renal caryces and extravasation of contrast medium around the left kidney. CT revealed that a tumor was located below the left renal hilus invading to the lower pole of the kidney. One week after admission, the patient complainted of a dyspnea. Chest X-P showed a significant fluid retention in the left pleural cavity. The pleural fluid obtained from thoracentesis was proved to be urine by biochemical analysis (Urinothorax).
