尿管結石による尿嚢腫

URINOMA CAUSED BY URETERAL STONE

　46歳，男性。右側腹部痛で来院し，KUBで右尿管結石と診断した。翌日IVPで右腎盂に造影剤の溢流像がみられた。2週後，超音波検査，CTにて右腎下方に尿嚢腫の形成が認められた。右尿管切石術と尿嚢腫の排液を行い尿嚢腫は消失した。本例は尿管結石による自然腎盂外溢流の結果，尿嚢腫が形成されたものであるが，このような尿嚢腫は稀である。

A 46-year-old man complained of right flank pain. Plain film of the abdomen revealed a small right ureteric stone. Excretory urography showed extravasation of contrast media around the right renal pelvis. Two weeks later, ultrasonogram and CT scan demonstrated a right infrarenal cystic mass exten-ding to the pelvic cavity. A diagnosis of urinoma was made. The patient underwent a successful drainage of the mass and ureterolithotomy. Urinoma caused by ureteric stone is rare.
