顕微鏡手術による陰茎海綿体・血管バイパス手術

MICROSURGERY REFORM OF VASCULAR BYPASS FOR PENILE CORPUS CAVERNOSUM

　陰茎海綿体・血管バイパス手術の成否は，血管・陰茎海綿体白膜開窓部の吻合の良否，すなわち吻合技術の精緻さいかんにかかっている。吻合部の血流率を高めて手術の成功を確実なものにするため，顕微鏡手術を用いて従来の手術法に改良を加え「白膜開窓新法」と「陰茎海綿体・血管バイパス腔支持吻合法」を開発した。1985年以来，血管性インポテンスと特発性陰茎持続勃起症の各2例に対して本改良術式を適用し，満足すべき結果を得た。

Key notes of vascular bypass for penile corpus cavernosum are to secure the patient fenestration in tunica albuginea of penile corpus cavenosum. In order to raise the percentage of successful anasto-mosis, the original operation was modified by utilizing microsurgery. Our new method also included a polyethylene tube as a holder in anastomosis. Four cases in priapism and in vascular impotence had been successfully treated by this new method since 1985.
