10年間の抗男性ホルモン療法後に発生した前立腺移行上皮癌

TRANSITIONAL CELL CARCINOMA OF THE PROSTATE DEVELOPING AFTER 10 YEARS'ANTI-ANDROGENIC THERAPY

　84歳，男性。10年前，前立腺腺癌と診断され，抗男性ホルモン療法で前立腺は萎縮し，無症状で経過していたが，6ヵ月前から切迫尿失禁と排尿終末時痛を訴えた。経過とともに前立腺右葉が硬く腫大してきたが，前立腺性酸フォスファターゼは正常であつた。TUR切片には移行上皮癌と腺癌の合併が認められた。症状再発後2年で死亡した。再燃癌に対しては，経尿道的生検が必要と思われた。

A 84-year-old man who had had prostatic adenocarcinoma and had been well controlled with an anti-androgenic therapy for 10 years complained of urge incontinence and terminal pain on urination of 6 months' duration. The right lobe of the prostate became firmly enlarged, but serum prostatic acid phosphatase remained within normal limit. Pathological examination of the resected specimens of the prostate revealed transitional cell carcinoma associated with adenocarcinoma. The patient died 2 years after relapse of symptoms.
