　気腫性腎盂腎炎は腎実質の広汎な壊死を来す重篤な感染症である。X線的には腎において腸管とは異なるガス像を呈することを特徴とする。各種抗生剤の発達した現在でも致命率は高く，外科的治療の適応となる場合が多い。われわれは糖尿病に合併した大腸菌と真菌の混合感染によると思われる気腫性腎孟腎炎の1根治例を経験したので報告し，自験例も含めた本邦報告24例を集計した。

A 61-year-old female was admitted with severe diabetes mellitus and urinary tract infection in September 1985. Low density areas were found in left renal parenchyma by CT. Angiography revealed no tumor and the patient was discharged by herself without permission in spite of bad control of blood suger. On January 25, 1986, she was admitted again with fever, chills and left lumbago. Hematopyuria, leucocytosis, thrombocytopenia, high level of FDP and E. coli from blood culture indicated DIC from urinary tract infection. KUB showed enlarged air nephrogram and CT indicated whole renal parenchyma was destructed.
