選択的腎動脈内注人線溶療法が有効と思われた腎動脈塞栓症の1例

A CASE REPORT OF RENAL EMBOLISM TREATED WITH ARTERIAL INFUSION OF FIBRINOLYTIC DRUG

緒言

　腎動脈塞栓症は決して稀な疾患ではないと言われながら^1），診断の困難さもあり，経験する機会の少ない疾患であつた。しかし，近年の診断技術の進歩，普及に伴い腎動脈塞栓症が早期に診断されることも多くなつてきた。一方，腎動脈塞栓症の治療法は以前は腎摘出術によることがほとんどであつた。しかし，近年では線溶療法などの保存的治療法，さらに血行再建術，塞栓摘出術などの積極的な外科的治療法も行われ，腎の機能を保存する方向へ進んでいる。

　われわれは，最近腎動脈塞栓症の1例に選択的腎動脈内注入線溶療法を施行し有効と思われたので，若干の考察を加え報告する。

A 34 year-old man with atrial fibrilation and hypertension, experienced an abrupt onset of severe pain in the right part of abdomen. Computed tomography and angiography showed emboli of segment arteries in right kidney. 60 hours after the onset, the patient was treated with renal arterial urokinase infusion. The infusion dose was 480,000 units in 20 minutes. For next 3 days, urokinase of 240,000 units was given into peripheral vein slowly.
