巨大副腎皮質癌の1例

A CASE OF GIANT ADRENAL CORTICAL CANCER

緒言

　副腎皮質癌は極めて稀な疾患で，文献的にも全癌の0.2％前後の頻度^1）とされている。ホルモン活性を有するものは特異な症状により早い時期に診断可能であるが，非活性例ではしばしば巨大腫瘤を形成しその予後も不良である^2）。今回われわれは内分泌症状を呈さず，左季肋部腫瘤を主訴とした本症の1例を経験したので報告する。

The patient was 70 year-old woman suffering from It. subcostal mass and pain. Hormonal symptomes were absent. Several steroid hormons were investigated. Only urine estrogen level was elevated slightly and urine 17-KS, 17-OHCS, aldosterone and blood aldosterone levels were within normal range.

The tumor, completely excised was huge and weighed 1,010g but not infiltrated to surrounding structures. Microscopic diagnosis was adrenal cortical cancer without lymphnodes metastases.

Prognosis of non-functional cancer of adreal cartex is very poor but patient is healthy at 8 months after surgery.
