重複腎盂尿管に発生した原発性尿管癌の1例

A CASE OF PRIMARY URETERAL CARCINOMA IN THE DUPLICATED URETER

緒言

　重複腎盂尿管は，奇形発生の頻度が比較的高い泌尿器系の中でも最も多く認められる奇形である^1）。その多くは臨床的に問題となることは少ないが，種々の合併症を伴つた場合，その診断および治療はしばしば困難となつてくる。今回，われわれは，右不完全重複腎盂尿管に原発性尿管癌の発生した興味ある1例を経験したので若干の文献的考察を加えて報告する。

A 62-year-old women presented with gross hematuria and right flank pain in May 1980. An excretory urogram (DIP) and renal scintigram strongly suggested the presence of a duplication of the right collecting system, with non-functioning lower half of the kidney. A retrograde ureteropyelogram revealed an incomplete duplication of the right ureter fused at about 3cm from the bladder and a complete occlusion of the lower segment of the duplicated ureter at about 10cm from the bladder. Computed tomograms provided a definitive information of a solid tumor filling the duplicated lower seg-ment of the ureter.
