褐色細胞腫におけるラベタロールの使用経験

EXPERIENCE WITH LABETALOL IN THE SURGICAL TREATMENT OF PHEOCHROMOCYTOMA

緒言

　褐色細胞腫の手術は，過剰輸血^1）による術後管理法の開発や，カテコ—ルアミン受容体遮断剤による術前処置^2）が普及するに従い，容易に行なわれるようになつてきている。しかし，それでもなお泌尿器科医の行なう手術の中では危険で難しいものの1つに挙げられている。その理由は，手術手技上の問題もさることながら，術前から術後にかけての血圧を中心とした循環系の管理が，現在でも完全とはいえないことによる。

　最近，α受容体遮断作用を併せもつβ受容体遮断剤labetalolが登場し，これが褐色細胞腫の高血圧のコントロ—ルに有効であることが示されている^3）。labetalolはイギリスのグラクソ社で開発された物質で，化学名は2-hydroxy-5-〔1-hydroxy-2-〔（1-methyl-3-phenylpropyl）amino〕ethyl〕benzami-deである。ヒトにおいては経口投与の場合，α受容体遮断効果とβ受容体遮断効果の比は約1：3であるとされている^4）。
