We experienced two cases of renal cell carcinoma with complications of varicocele. Both cases had gigantic arteriovenous fistula at the tumor site, arterial blood flowing into the renal vein, thus, an elevated renal venous pressure. It was postulated that the rise in renal venous pressure increased testicular venous pressure, which resulted the varicocele in a retrograde fashion.

The pathogenesis of varicocele associated with renal cell carcinoma has been thought to the obstruc-tion of testicular vein by the tumor, however, we assumed that elevation of testicular venous pressure is the trigger mechanism by which varicocele developes.
