精系静脈瘤を合併した腎癌の2例—精系静脈瘤の成因について

TWO CASES OF RENAL CELL CARCINOMA WITH COMPLICATIONS OF VARICOCELE: THE PATHOGENESIS OF VARICOCELE

緒言

　腎癌に精系静脈瘤を合併することは古くからしられている。その成因は腫瘍栓塞もしくは周囲癌組織の圧迫による睾丸静脈の閉塞によるとされている。最近2例の精系静脈瘤を合併した腎癌を経験したが，睾丸静脈の閉塞は認められず，むしろ腎腫瘍内の巨大な動静脈瘻により腎静脈圧が上昇し，睾丸静脈に波及したことが，精系静脈瘤の成因と考えられた。腎癌における精系静脈瘤の成因について考察を加え症例を報告する。

We experienced two cases of renal cell carcinoma with complications of varicocele. Both cases had gigantic arteriovenous fistula at the tumor site, arterial blood flowing into the renal vein, thus, an elevated renal venous pressure. It was postulated that the rise in renal venous pressure increased testicular venous pressure, which resulted the varicocele in a retrograde fashion.

The pathogenesis of varicocele associated with renal cell carcinoma has been thought to the obstruc-tion of testicular vein by the tumor, however, we assumed that elevation of testicular venous pressure is the trigger mechanism by which varicocele developes.
