緒言

　化膿性孤立性腎嚢胞は，欧米では1917年，Cassioliにより最初に発表されたが，本邦では1953年の近藤の報告^1）が初例であり，以後斯波ら^2）の1967年の集計報告を含めて，6例の記載^3〜6）を認めるにすぎない。最近われわれはその1例を経験したので，若干の考察を加えて報告する。

A 26-year-old man was admitted to our hospital on February 7, 1978 complaining of a 2-day history of fever, chills and right flank pain of unknown etiology. He denied previous urinary tract symptoms.

On physical examination, he was found to have muscular rigidity and tenderness in the right flank, a pyrexia of 38℃, a pulse rate of 114/min., and blood pressure of 130/80mmHg.

According to laboratory findings, white blood cell count was 21,700/mm³ with a shift toward imma-ture cells and erythrocyte sedimentation rate was 81mm/1h. Several leucocytes were found in the urine but cultures proved negative.
