右腰部打撲による左先天性水腎症の腎外傷

A CASE REPORT OF GIGANTIC CONGENITAL HYDRONEPHROSIS FOUND FOLLOWING CONTRALATERAL FLANK TRAUMA

緒言

　病的腎は正常腎に比し，腎外傷を起こしやすいことはすでに多くの指摘があり，水腎症に腎外傷を発症した例は，本邦でも過去三十数例の報告がみられる。最近われわれは先天性水腎症に腎外傷を起こした症例を経験したが，その直接原因は患側腎の反対側腰部打撲によるものであつた。このような例は本邦文献上まだ報告を見ないようであり，本症例の起始および経過について報告するとともに，本邦における水腎症に発生した腎外傷の統計的検討を加える。

A 37-year-old male was admitted to the hospital because of hematuria following abdominal blunt injury. He fell down while skating and had contusion on the right flank. He had severe contralateral flank pain followed by gross hematuria and high fever. A mass of an infant-head-size was palpated on his left flank with mild tenderness. Renal trauma was suspected, but he appeared to be well. In-travenous pyelogram showed a normal right kidney, nonvisualized left kidney with a huge mass in the left hemiabdomen. Angiography failed to demonstrate the left renal artery.
