下腹部腫瘤を契機に診断された前立腺癌

Prostate cancer presenting a huge abdominal mass：a case report

82歳男性がCTで骨盤腔を占拠する下腹部腫瘤を指摘され受診。PSAが32,110ng/mlと異常高値のため，腹部腫瘤生検と前立腺針生検の結果，グリソンスコア4＋4前立腺癌と診断。下腹部腫瘤は骨盤内リンパ転移と診断。ホルモン治療を行い，1年4か月後PSA，画像上，どちらも良好な反応を示している。

In 82-year-old man with huge lower abdominal mass. CT revealed the mass occupying the pelvic cavity. Pathological diagnosis of the biopsy specimens from the abdominal mass and the prostate showed adenocarcinoma, with Gleason score 4＋4. The abdominal mass was the lymph-node metastasis from the prostate cancer. A follow-up CT revealed disappearance of the huge lymph-node metastasis and decreased PSA to the normal limit after the sixteen months hormonal therapy with combined androgen blockade.
