無痛性硬結を呈した小児精巣原発類表皮囊胞

Epidermoid cyst with painless induration from testis of pediatric patient：A case report

症例は11歳の男児で，右精巣内に無痛性の硬結を触知し当科を受診した。血液検査では腫瘍マーカーの上昇は認めず，腹部骨盤CTでも異常は指摘されなかった。触診，超音波検査上精巣腫瘍と診断し，緊急手術施行した。術中迅速病理検査の結果，類表皮囊胞と診断。腫瘍核出術を行い，精巣を温存できた。小児精巣腫瘍においては，類表皮囊胞も念頭に置き，手術適応の判断を行う必要があると考えられた。

An 11-year-old-boy was brought to our hospital with right scrotal swelling without pain. Serum tumor markers were within normal range. Palpation and ultrasonography revealed a testicular tumor and computed tomography showed no metastasis. He immediately underwent an operation for his testicular tumor. Because the frozen section examination revealed the epidermoid cyst,we performed an enucleation of the tumor. The final pathological diagnosis was epidermoid cyst. In prepubertal testicular tumor patients,the enucleation of the tumor,should be considered if confirmed as benign tumor by the frozen section examination. Management of testicular tumors in prepubental patients was discussed.
