両側肺気胸に合併した陰囊内気腫

Pneumoscrotum follwed by bilateral pneumothorax：A case report

　症例は65歳，男性。頸椎損傷後四肢麻痺，神経因性膀胱に対して膀胱瘻による排尿管理を続けていた。64歳時に肺炎を併発，以降在宅レスピレーターによる呼吸管理を施行していたが，2007年10月，呼吸困難を生じ来院した。両側肺気胸，腹腔内気腫，皮下気腫および陰囊内にも気腫を認めた。両側肺気胸に対して胸腔ドレナージ施行後，陰囊内気腫は消失した。

　The patient was a 65-year-old man traffic accident with a hemiparalysis and neurogenic bladder caused by at the age of 43. At age 58,he underwent a cystostomy for the control of the UTI. Because of poor respiratory function he had been controlled by the respirator in his home from the age of 64. He was referred to our hospital with severe dyspnea in October 07,and was diagnosed as the bilateral pneumothorax,retroperitoneal emphysema and pnumoscrotum. He was immediately admitted and underwent the drainage of for bilateral pneumothorax. After the drainage pneumoscrotum disappeared as well.
