経尿道的切除術を受けた男性77症例を対象に,術前および術後2か月目と6か月目にIIEF5を中心とした性機能アンケート調査を行った。対象の平均年齢は73.3歳で,術前に性生活があった症例は13例(17%)であった。TUR-Pの39症例およびTUR-Btの38症例のいずれにおいても,術前後のIIEF5に変化は認められなかった。術前IIEF5が5点以上であった症例に限れば,TUR-Btの14症例では術前後でIIEF5に変化はなかったものの,TUR-Pの16症例では14.6±5.5から2か月目11.3±8.7,6か月目14.4±7.4と,2か月目に有意な低下(p=0.03)が認められた。TUR-P後に一時的に性機能障害が生じる可能性が示唆された。
The sexual activity was prospectively assessed in patients who underwent TUR-P or TUR-Bt between January 2006 and March 2007. We evaluated sexual function of patients using questionnaires including the International Index of Erectile Function Instrument(IIEF5). The questionnaire was performed before surgery, and 2 and 6 months after surgery. Seventy-seven patients(TUR-P:39, TUR-Bt:38)were enrolled in the survey. The mean age was 73.3 years. Before surgery, 17% of the patients were active in sexuality and the mean IIEF5 was 5.8. IIEF5 did not demonstrate significant changes after TUR-P and TUR-Bt. However the patients with IIEF5 over 5 before surgery, showed a significant decrease at 2 months after TUR-P(p=0.03), while no significant change was observed after TUR-Bt. From our study it was suggested that TUR-P provides some influence on sexual function.
