経過中に2度のデブリードメントを要したフルニエ壊疽

Fournier's gangrene requiring debridement twice during the course

　2度のデブリードメントを要したフルニエ壊疽の2例を経験した。1例目は糖尿病が背景にあり，2度にわたるデブリードメントののち植皮を行った。2例目は同じく2度のデブリードメントを行ったが，尿道まで創部が進展してしまい，尿道狭窄をきたし，尿道カテーテル留置管理となった。フルニエ壊疽は急激に進行し予後不良のケースもあるが，今回の2症例は泌尿器科と形成外科の連携をはかることにより重症化を未然に防ぐことができた。

　We experienced 2 patients with Fournier's gangrene that required debridement twice. One patient, 34-year-old male with diabetes mellitus underwent debridement twice followed by skin grafting. The other patient, 63-year-old male underwent debridement but developed urethral involvement and stricture and was managed by catheter insertion. Fournier's gangrene acutely progresses, resulting poor outcome in some cases. In these two patients, the aggravation of the disease could be prevented by cooperative treatments performed by urologists and plastic surgeons.（Rinsho Hinyokika 62：151-153, 2008）
