要約　75歳，女性．初診の3年前より両側頸部・胸部・腋窩に瘙痒を伴う黄白色の小丘疹が集簇多発してきた．精査にて眼病変，心血管病変の合併はなく．病理組織像では真皮乳頭層で弾性線維が減少している以外，特記すべき所見は認めなかった．以上よりpseudoxanthoma elasticum-like papillary dermal elastolysis(PXE-like PDE)と診断した．ダーモスコピーでは淡く境界不明瞭な黄白色の無構造領域が集簇・融合してみられた．臨床的にpseudoxanthoma elasticum(PDE)やwhite fibrous papulosis of the neck(WFPN)との鑑別が時に問題となる．PXE-like PDEのダーモスコピー像は，淡く境界不明瞭な黄白色の無構造領域である一方で，WFPNでは境界明瞭で均一な白色領域が存在する．PXEのダーモスコピーでは黄白色の無構造領域が粗大網状に融合し，健常部は淡紅色を呈する．
