皮膚刺激反応に影響する諸因子

職業性皮膚疾患と皮膚刺激反応

　接触皮膚炎は発症機序の面から刺激性，アレルギー性接触皮膚炎に2大別され，もしもphotocontact derm-atitisも広義の接触皮膚炎と見なすならば，光中毒，光アレルギー反応についても考慮せねばならない。皮膚科領域でしばしば問題にされ，あるいは研究対象になつているのは，これらのうちとくにアレルギー性あるいは光アレルギー性接触皮膚炎であるが，刺激性接触皮膚炎も実際にはかなりの症例数があることが推測される。

　皮膚刺激反応がとくに問題になるのは職業性皮膚疾患の場合である。すなわち，Weinberger^1）（1950），Birmingham^2）（1957）は職業性皮膚疾患の80％が1次刺激物質により発症していると報じ，Klauder^4）（1962）も27.5％が刺激反応，16.2％がアレルギー反応という統計数値を示した。主婦湿疹も広義の職業性皮膚疾患の範疇に入ると考えられるが，その発症機序について，Skog^5）（1959）は63％が皮膚刺激反応，37％が皮膚アレルギー反応であつたと述べ，Agrup^6）（1968）も60％が刺激反応によるとした。著者^7）もいわゆる進行性指掌角皮症（KTPP）型の主婦湿疹の発症・増悪に，水，石鹸，洗剤，漂白剤などの洗浄剤や機械的刺激による皮膚脂質の除去，角質層の障害を重視，KTPPは1種の刺激性皮膚炎であると考えている。ただしKTPP以外の手の皮膚炎には，ニッケル，クロームなどの金属，ゴム，フォルマリン，その他のアレルギー反応で発症している例も少なくない^8）。

To evaluate the actual role of mild irritants on the non-allergic contact dermatitis, occupa tional dermatoses, housewife's eczema and patch tests, many factors should be taken into con sideration, especially in some kinds of irritants,-cumulative effects of the irritants, influences of the environmental factors to the irritant effects, conditions of the skin, how to make a conta-ct, and various local characteristics of the skin.

The following results were found ; 1) Non-ionic detergent and propyrene glycol have irritant effect under the ; moisture condition. lower 2) Perfumes from Citrus fruits have irritant action under the high temperature and humidity environment. 3) Propyrene glycol have the irritant effect to the eczematous lesion.
