抗結核剤（INAH, Ethionamide）によるMultiple itamine Deficiency Stateの1例

MULTIPLE VITAMINE DEFICIENCY STATE DUE TO INAH AND ETHIONAMIDE

はじめに

　Isonicotinic acid hydrazide （以下INAH）によるビタミンB₆の欠乏，精神神経症状は，その副作用としてよく知られている。しかし，INAHがニコチン酸誘導体の一種であるためニコチン酸と競合いを生じその結果，ニコチン酸の欠乏を生じ，ペラグラに類似した症候を呈してくる事は，理論としてうなづける事ではあるが，いまだその症例報告が，はなはだ少ないためもあつてあまり知られていない。著者は最近，INAHと，同じくニコチン酸誘導体であるエチオナマイド（ethionamide）によつて生じたペラグラ類似のビタミン欠乏性皮膚疾患を経験したので以下に報告する。

A 21-year-old woman developed pellagra-like lesions on the hands and feet, an acneiform eruption and seborrheic scaling on the face during the treatment of pulmonary tuberculosis with INAH and Ethionamide. After about 2 weeks' treatment, with nicotinic acid, the patient showed remission.

Antituberculous agents with a similar structure of nicotinic acid may show a deficiency syndrome as a result of competitive inhibition.
