要約　70歳，女性．後頭部に10年前に生じ，漸次増大した約10mm大で軽度隆起し，常色弾性硬の皮下結節で下床との可動は良好であった．MRIでは囊腫壁の一部に不整な肥厚を認めた．病理組織像では真皮内に囊腫構造があり，MRI画像と一致して一部に壁が肥厚していた．肥厚部では大小の囊腫構造が融合し索状・網目状に増殖．増殖部分の細胞には軽度の核異型がみられた．囊腫壁は小型の基底様細胞と内腔に向かって大型化し，顆粒層を欠いて角化する細胞で構成されていた．以上より，proliferating trichilemmal cystic acanthoma(PTCA)と診断した．自験例と当科で経験したtrichilemmal cyst(TC)，proliferating trichilemmal tumor(PTT)，malignant proliferating trichilemmal tumor(mPTT)とともに，MRI像と病理組織像を比較検討した．PTCAの病理組織像とMRI像をTC，PTT，mPTTと比較すると，壁肥厚と造影所見がよく対応した．これらを連続性病変と考えるとMRI像は病理組織構築をよく反映しており，有用な術前検査である．
