A 50-year-old female complained of vertigo with acute hearing loss and tinnitus. Otherwise, she had no obvious neurological symptoms. She showed spontaneous horizontal and rotatory nystagmuswhich seemed to be caused by a peripheral vestibu-lar disorder.

After 5 days, she showed disequilibrium, mild hypesthesia of the face and dysarthria. MRI reveal-ed a lesion suggesting anterior inferior cerebellar artery infarction.

The nystagmus changed to gaze nystagmus dur-ing the clinical course.

Neurological examinations and MRI are impor-tant for distinguishing cerebellar infarction from inner ear disease.
