血管造影にて血管腫が疑われた副咽頭神経鞘腫

A Case of Parapharyngeal Schwannoma, Suspected of Hemangioma by Angiography

　はじめに

　副咽頭間隙は，上方は頭蓋底，後方は椎前筋膜，前方は内側翼突筋，側方は耳下腺深葉，下方は舌骨，内側は上咽頭収縮筋に囲まれている。解剖学的にいくつもの神経，血管が複雑にその内部を走行し，腫瘍がある程度大きくならないと発見されにくい場所である。今回われわれは，病理組織学的に海綿状血管腫様変化をきたしたため，術前の塞栓術が必要であった巨大な副咽頭神経鞘腫の1例を経験したので報告する。

A 35-year-old male patient was suffering from alarge parapharyngeal schwannoma which was ini-tially suspected of hemangioma after selective an-giography showing a vascular lesion similar to hemolymphangioma. After embolization, the tumor was removed by cervical approach with midline mandibulotomy. Histologically, it was diagnosed as schwannoma with increased angiomatous compo-nents.
