A 55-year-old male presented with a 30-years' history of vertigenous attack accompanied with bilateral tinnitus and hearing loss. Glycerol test and electrocochleogram indicated bilateral endolym-phatic hydrops. Hyperbilirubinemia was found by serological examination. Serum indirect bilirubin was correlated to the hearing level ; it increased when his right hearing was good and decreased when it was poor. We assumed that the underlying pathology of this case was endolymphatic hydrops due to a microvascular disease which caused intra-vascular microhemolysis.
