Ⅰ.はじめに
後下小脳動脈(posterior inferior cerebellar ar-tery:PICA)に限局する解離性動脈瘤は報告例が少なく,その自然経過も不明な点が多いため,治療方針は各症例ごとに検討されているのが実状である.
今回われわれは虚血症状で発症したPICA解離性動脈瘤に対して保存的治療を行い,慢性期において動脈瘤の消失を画像的にとらえた症例を経験したので,自然経過と治療方針を中心に若干の文献的考察を加え報告する.
We report here a case of a patient with a dissecting aneurysm of the anterior medullary segment of the posterior inferior cerebellar artery (PICA) which presented with Wallenberg's syndrome. A 32-year-male presented with an unusual case of Wallenberg's syndrome due to a dissecting aneurysm of the PICA manifesting as a sensation of heaviness in the occipital region and vertigo. The occipital symptoms persisted and vertigo and vomiting developed after 6 days. Numbness developed on the left side of the patient's face, and hy-peralgesia on the right side of the body. The diagnosis of Wallenberg's syndrome was based on the above findings.
