　はじめに

　Wegener肉芽腫症をはじめとする一連の血管炎症候群において，血清中の好中球細胞質に対する自己抗体，すなわち（anti neutrophil cytoplas-mic antibody：ANCA）の存在が認識され，特にC （cytoplasmic pattern）—ANCAはWegener肉芽腫症，P （peri-nuclear pattern）—ANCAは顕微鏡的多発動脈炎などの細動脈以下の血管炎を示す疾患に特異性が高いといわれ，その臨床的意義が広く注目されている^1〜6）。

　今回，われわれはC-ANCAの測定結果が早期診断および治療効果の判定に有用であったWegener肉芽腫症の1症例を経験したので若干の文献的考察を加えて報告する。

A 43-year-old male presented with a complaint of nasal obstruction resistant to treatment for 3 months.

There were saddle nose and a large amount of purulent crust on nasal mucosa. Wegener granulomatosis was diagnosed by the presence of cytoplasmic (C) ANCA (27.5 EU) and characteris-tic pathological finding. After the disease activity was controlled, C-ANCA became negative.
