Wegener肉芽腫症におけるHLAの関与について

　はじめに

　Wegener肉芽腫症（以下WGと略す）は原因不明の上気道と肺の壊死性肉芽腫性炎，全身の壊死性血管炎および巣状壊死性腎炎を三徴とする疾患である。WGの病因は現在のところ不明であるが，抗好中球細胞質抗体（anti-neutrophil cytoplasmic antibody：ANCA）の存在^1），血清IgE^2）およびIgAレベルの上昇^3），抗平滑筋自己抗体の存在^3），臨床的に免疫抑制剤やステロイドが奏効することなどより何らかの免疫学的機序が成因に関与しているのではないかと考えられている^4）。

　一方，HLA抗原は免疫学的認識反応において抗原提示の際に重要な役割を果たしており，自己免疫疾患の発症に関与していると考えられている。事実，ある種の自己免疫疾患において特定のHLA抗原との関与が認められている。

　そこで今回われわれは，WGの発症にHLAが関与しているかどうかを調べるため，当科通院中のWG患者におけるHLAのタイピングを行い正常者のHLAの頻度と比べてみた。

The frequency of HLA antigens was determined in 11 patients with C-ANCA positive Wegener's granulomatosis and compared with their frequencyin healthy Japanese control population. An increased frequency of HLA-DR9 was found in patients with Wegener's granulomatosis. However, it is difficult to say that HLA DR9 is directly involved in pathogenesis of Wegener's granulomatosis because a number of patients was small.
