鼻腔悪性黒色腫のスタンプ螢光法ならびにガーゼ滲出液中5-S-Cysteinyldopa測定による術前診断

Diagnosis of Malignant Melanoma in the Nasal Cavity with Touch-fluorescence Method and Detection of 5-S-Cysteinyldopa

　緒言

　悪性黒色腫の診断は，通常臨床的観察によってなされるので，黒色腫でないのに広範囲切除術，黒色腫であるのに小切除が行われる恐れがある。皮膚原発はもちろんのこと，他部位原発の悪性黒色腫でも生検は禁忌と考えられ，一般化された術前の確定診断がない現在，誤診例は稀ならず存在するものと思われる。このような誤診例を避けるため，われわれはメラニン代謝の主要中間代謝産物である5-S-Cysteinyldopa （5-S-CD）の組織化学的証明や化学的定量による，主として皮膚の悪性黒色腫の新しい診断法の開発を志してきた。組織化学的証明法であるスタンプ螢光法，すなわち病巣表面からの本法^1）は術前の診断法として，病巣割面からのそれ^2）は術中迅速診断法として極めて有用である。そして，化学的定量法としての尿中5-S-CD値の測定^3,4）は黒色腫の腫瘍マーカーとして，ガーゼ滲出液中5—S-CD値の測定^5）は腟の黒色腫の診断に役立つことなどである。

　今回，われわれは，鼻腔の色素性腫瘤が悪性黒色腫であるか否かの確定診断の依頼を受け，病巣表面からのスタンプ螢光法およびガーゼ滲出液中5-S-CD値の測定が鼻腔悪性黒色腫の術前診断に極めて有用であったので，ここに紹介したい。

A 58-year-old man presented with a melanotic tumor in the right nasal cavity. Touch fluoresencemethod and detection of 5-S-Cysteinyldopa (5-S-CD) in a sponge gauze were used to diagnose malignant melanoma preoperatively. The fluores-cence tumor cells were detectable from the surface of the lesion, and 5-S-CD in the gauze showed a high level of 1114.8 ng/PCA ml. The 5-S-CD levels fluctuated in parallel with the therapeutic efficacy. It was concluded that 5-S-CI) could be a tumor marker of nasal malignant melanoma as well as the cutaneous ones.
