ロック難聴と急性音響外傷の臨床的検討

Rock Music Hearing Loss and Acute Acoustic Trauma: A Clinical Report

　はじめに

　1950年代後半に出現したロックンロール（ロック）は，電子楽器を用い，強いアフタービート，社会性のある歌詞，そして強大な音を特徴とする^1）。1962年のビートルズの登場とともに世界中に飛躍的に広がるが，時を同じくするように，諸外国では1960年代後半より，強大な音響による急性難聴（ロック難聴）の発症が認められるようになった^2,3）。わが国では1970年代後半より報告がみられ^4〜9），1986年には全国的なアンケート調在をもとに120症例が報告されている^10）。これらの音楽聴取による難聴は，立木の分類^11）における，予期している強大音の短時間負荷による難聴に相当すると考えられ，爆発音などの予期しない突発的強大音による急性音響外傷とは臨床像や機序に異なる点があるとされる。今回，あるロックコンサートを聴取した後に発症した，ロック難聴の3症例を経験したので，その臨床経過を提示した。また，過去に当科を受診した他のロック難聴症例および急性音響外傷症例とをあわせて，臨床像を検討したので報告する。

The purpose of present study was, firstly, to report the clinical courses of temporay hearing loss in three young women who attended a rock-and-roll music concert. and secondly to study eighteen cases of rock music hearing loss and eight cases of acute acoustic trauma retrospectively.

In the cases of rock music hearing loss, the unilateral or bilateral ears were suffered regardless of the direction of sound source. The subjective symptoms were tinnitus, fullness of ear and de-creased hearing with C₅ dip and 2,000 Hz dip. The hearing level in the younger subjects returned to normal within one week, while a permanent hearing loss was observed in the elderly subjects.

In the majority of cases of acute acoustic trauma, unilateral car close to the sound source was affe-cted irreversibly and non-characteristic audio-grams were obtained.
