頭位性めまいで発症した頭蓋内副神経鞘腫の1例

Intracranial Accessory Nerve Neuroma Accompanied with Positional Vertigo:Report of a Case

　はじめに

　頭蓋内原発性腫瘍の約9％は小脳橋角部腫瘍が占め，その多くは第Ⅷ脳神経鞘腫であることは広く知られている。一方，第Ⅷ，V脳神経以外の神経鞘腫に遭遇することは比較的稀なことである。今回，われわれはめまいで発症した巨大な頭蓋内副神経鞘腫を経験したので，その神経耳科学的所見を中心に報告する。

A 19-year-old female complained of positional vertigo with vomiting for a year, a left hearing impairment, facial numbness and headache. Ope-ration revealed that main portion of the tumor was located in the left cerebello-pontine cistern and was connected to the eleventh cranial nerve. Pathological diagnosis was neurinoma. Review of the literature showed that intracranial accessory nerve neuromas are very rare. Unexpectedly, symptoms involving the eighth cranial nerve are more frequently than those of the eleventh cranial nerve.
