前庭性片頭痛確実例における五苓散とロメリジン塩酸塩の併用治療—気象病関連症状の有無による治療効果の差の検討

Therapeutic effects of lomeridine hydrochloride and Goreisan in some cases of vestibular migraine

はじめに

　頭痛やめまいは気象病関連症状^1〜3）と考えられている。五苓散は慢性頭痛の診療ガイドライン2013^4）から明記され，頭痛とめまいの保険適応を有し，さらに気象病関連症状に効果があるとの報告^1〜3）を認める。

　一方，片頭痛を本態とし，めまいを認める前庭性片頭痛^5）患者は気象病関連症状と酷似している。前庭性片頭痛の代表的治療にはCa拮抗薬（ロメリジン塩酸塩：以下ロメリジン）を用いた予防治療がある^5,6）。そこで，本研究では前庭性片頭痛患者にロメリジンと五苓散^7）を併用治療した。その治療下で気象病関連症状を有する患者が有しない患者より片頭痛改善効果が認めやすい（＝レスポンダー）ことを確認した。本研究は横浜市立みなと赤十字病院医療倫理委員会 承認番号2022-28を得て実施した。

　Subjects and Methods：Thirty-seven cases of vestibular migraine were treated with lomeridine hydrochloride 20 mg/day and Goreisan 7.5 g/day.

　Results：Results showed improvement in HIT-6 and DHI one month after treatment（p＜0.01）. Headache and dizziness were considered meteorology-related symptoms. Twenty-six patients with subjectively meteorological illness-related symptoms witnessed improvement in HIT-6 and DHI（p＜0.01）, while 11 patients with no symptoms did not show improvement in HIT-6.

　Summary and discussion：This study highlights the potential efficacy of Goreisan in managing migraine with meteorological illness symptoms.
