 Spontaneous perilymphatic fistula is difficult to diagnose and the associated hearing loss is not easy to treat. A 52-year-old woman presented with hearing loss and vertigo. A cochlin-tomoprotein(CTP)detection test and operation were performed 15 days after the onset of symptoms. She was positive for CTP. The detection of CTP in the middle ear has been included in the revised definitive diagnostic criteria for perilymphatic fistula in Japan. The patient was therefore diagnosed with definitive perilymphatic fistula.

 The postoperative course was uneventful, hearing loss and vertigo improved after the procedure.
