急性副鼻腔炎に対するLevofloxacin 1日1回500mg投与の治療効果

Clinical efficacy of once daily administration of 500 mg levofloxacin in treatment of acute rhinosinusitis

Ⅰ．はじめに

　近年，耳鼻咽喉科および呼吸器科領域において耐性菌の増加が問題となっており，抗菌薬の選択肢が狭まりつつある^1）。Levofloxacin（LVFX）は上気道および下気道感染症の主要原因菌に強い抗菌力を有し，わが国においても汎用されてきた。近年，いくつかの抗菌薬は，Pharmacokinetics-pharmacodynamics（PK-PD）理論に基づき，投与方法が見直されている^1）。濃度依存性に殺菌作用を示すニューキノロン系抗菌薬は，1回の投与量を増やしC_maxを上昇させることが効果の増強や耐性菌の出現防止に有効とされており，LVFXは新用法として500mg 1日1回投与が承認された^1）。

　本研究の目的は急性副鼻腔炎に対するLVFX 500mg 1日1回投与の有効性，特に早期治療効果と日本人における安全性を検討することである。

　A clinical study of once daily administration of 500 mg Levofloxacin（LVFX）in adults with acute rhinosinusitis was performed. We assessed LVFX efficacy on acute rhinosinusitis with local findings and clinical symptoms. The overall clinical efficacy was 84.6％ in acute rhinosinusitis. Bacteriological efficacy was 90.0％. After treatment, shadows of paranasal sinuses were disapeared in 66.7％ patients of acute rhinosinusitis. There was no serious or clinically significant event during and after treatment. In conclusion, once daily 500 mg LVFX treatment is useful and safe in acute rhinosinusitis.
