突発性難聴74例の入院治療の検討

A study of treatment on 74 inpatients of idiopathic sudden sensorineural hearing loss（ISSHL）

Ⅰ．はじめに

　突発性難聴（idiopathic sudden sensorineural hearing loss：ISSNHL）は，1944年にDe Kleyn^1）が21症例をはじめて報告した疾病である。その病因は感染説，血管障害説，代謝障害説などが挙げられてきているが，いまだに解明されていない^2）。したがって，エビデンスのある有効な治療は唯一副腎皮質ステロイドの投与のみであり^3），十分な治療法が確立されたとはいえない。よって，治療法は施設間あるいは個々の医師によってまちまちであるのが現状である。

　今回の検討の対象は，同一の主治医がほぼ同じ治療方針で治療を続けてきた74症例であるが，ここから得られた結果をふまえて，突発性難聴の病態を解析し，より的確な治療を行うことが本論文の目的である。

　74 inpatients of idiopathic sudden sensorineural hearing loss treated with administration of corticosteroids were studied.

　The percentage of Grade 3-4 hearing loss was 68.9％. The distributions of types of hearing loss were almost similar to that classified in published report.

　The complete recovery of hearing loss was 25.7％ as a whole, 33.3％ for Grade 1, 35.3％ for Grade 2, 26.5％ for Grade 3, 11.8％ for Grade 4, and there was no significant difference.

　Multivariate analysis in cure and non-cure groups revealed significant differences on age and time between onset and treatment start,but not on sex,grades,vertigo,diabetes mellitus,kinds of corticosteroids and SGB.
