挿管後に出現した広範囲な気管狭窄に対するダイナミックステント治療例

Treatment with a dynamic stent for wide spread tracheal stenosis occurred after oropharyngeal intubation

Ⅰ　はじめに

　気管狭窄は呼吸困難が出現するため，患者のQOLは著しく低下する。また，その原因，位置，長さ，狭窄部位の病理によっては，治療は難渋し長期間にわたることも多い。

　気管狭窄の治療の一つに，ステントを使用する方法がある。文献上では，悪性腫瘍で生じた気管狭窄に気管内ステントを留置する報告が散見される^1,2）。しかし，良性病変による広範囲な狭窄にダイナミックステントを使用した報告は稀である。

　今回は，気管内挿管後に出現した広範な気管狭窄に対し，気管支鏡下にKTP/YAGレーザーによる狭窄除去とダイナミックステントの気管内留置を行い，良好な結果が得られた症例を報告する。

　A 21-year-old man was treated for tracheostenosis with a Y-shaped dynamic stent（YDS）. His tracheostenosis of about 10 cm-in-length from the subglottic space near to the tracheal bifurcation,developed due to oropharyngeal intubation of 11 days during conservative therapy for subarachnoid hemorrhage. YDS was placed in the trachea and both main bronchi after vepolization of the stenosis with a KTP/YAG laser following useless treatment using silicone T-tube for 23 months. The YDS was broken and removed after 16 months. In conclusion,it seemed that treatment of tracheaostenosis with YDS was effective because tracheal restenosis has not occurred to date.
